Position is not a Testing Designated Position within the definition of the CSKT Drug Free Workplace Policy effective January 1, 1998. The successful applicant, if not already employed by the Tribes must pass a pre-hire drug test and serve a mandatory six (6) month probationary period.


The Community Health Registered Nurse conducts a continuous and comprehensive practice providing primary, secondary, and tertiary preventative nursing interventions to individuals, families, and communities. Services include but not limited to: assessment of the multiple determinants of health, monitoring of health status, diagnosis and investigation of community health problems and hazards, informing and educating individuals and populations, chronic and acute disease management, linkage of individuals to health services, hospital discharge planning, assessing for durable medical equipment needs, self-management support, and mobilizing community partnerships and actions.


The Community Health Registered Nurse provides leadership and supervision for a community health team consisting of both professionals and paraprofessionals. This includes but is not limited to the following functions:


  • Supervision of professional and paraprofessional staff per organizational chart according to accurate position descriptions, standards of performance and performance evaluations; implementing corrective plans and taking disciplinary action when appropriate; ensure proper orientation and training of staff in compliance with CSKT Personnel regulations and guidelines.
  • Implements, monitors and audits Nursing Division policies and procedures and appropriate standing orders ensuring compliance with applicable CSKT, federal and state laws and / or regulations. Recommends changes as necessary to maintain compliance and to meet the needs of the TH patients to provide a high quality of care.
  • Ensures compliance with CSKT and TH financial management standards including procurement policies and capital equipment acquisition. Complies with processes for expenditure of funds.
  • Complies with internal controls and conflicts of interest standards.

The Community Health Registered Nurse assists TH patients, families and communities to achieve and maintain their highest quality of health grounded in CSKT tribal values.


The Community Health Registered Nurse utilizes evidence-based strategies to promote the health of individuals and populations and prevent disease and injury.


The Community Health Registered Nurse conducts continuous surveillance and assessment of the community health status and evaluates health trends and risk factors of population groups.


The Community Health Registered Nurse visits individuals, families and community groups in various settings such as homes, jails, schools, hospitals and clinics to provide patient care and education/instruction in preventative, curative and rehabilitative health matters.


The Community Health Registered Nurse plans, implements, evaluates, and modifies nursing care plans based on subjective and objective data, collaborating with members of the primary care team, other health care professionals and service providers to ensure optimal patient care. The Community Health Registered Nurse coordinates case conferences so all agencies involved can plan for the patient’s continuity of care.


The Community Health Registered Nurse assists patients to access appropriate health/medical services and links patients to medical clinicians, counselors, emergency rooms, home health agencies, fitness centers, other tribal departments, transportation services, etc.


The Community Health Registered Nurse conducts communicable disease investigation, education, and follow-up. The Community Health Registered Nurse conducts epidemiological investigations during communicable disease outbreaks and coordinates these activities with the federal, state and county epidemiology staff, as indicated.


The Community Health Registered Nurse is the immunization point-of-contact for their respective community. The Community Health Registered Nurse monitors vaccination rates of populations and plans and participates in immunization campaigns.


The Community Health Registered Nurse is responsible for leading and developing programs in alignment with the ten essential services of public health that have potential to impact the entire service delivery area.


The Community Health Registered Nurse is responsible for initiating and maintaining collaborative partnerships with other Tribal Departments, government organizations, private organizations, and non-profit organizations.

  • Bachelor of Science Degree in Nursing from an accredited college of nursing.
  • Three years of full-time nursing experience providing direct patient care, with at least one year of Public Health or Community Health Nursing. Home health nursing experience may be considered.
  • At least two years supervisory experience, managing professional healthcare staff
  • A background to manage and supervise a broad range of public/community health nursing skills and public health programs
  • Experience using electronic data patient management systems, Microsoft office, including use in effective presentations and teaching
  • Valid and unrestricted Registered Nurse license in the state of Montana or compact state.
  • Public Health Nurse certificate, within 6 months of employment
  • Basic Life Support (BLS) for healthcare providers certification, Heart Saver CPR does not qualify
  • Valid Montana Driver’s License with ability to operate a vehicle

  2. Copy of relevant academic transcripts
  3. Copy of licensure.
  4. Completed background supplemental questionnaire.
  5. Proof of enrollment from a federally recognized Tribe if other than CSKT.
  6. If claiming veteran’s preference, a copy of DD214 must be submitted.
